Add Pro Se Option

User has requested that we add the option to include a pro se signature line for attorneys who prepare forms or pleadings for pro se parties on a limited scope services basis. This would involve including a new "checkbox" on the Staff on Matter dialogue (see attached screenshot) and modifying the signature block where appropriate.
